1. Shelter on the Hill provides applicants with vouchers by mail that will cover $25.00 of the cost of spay/neuter surgery only.
2. There is a limit of three vouchers per household per calendar year. The vouchers are only available to residents of 93222, 93225 and 93243 zip codes, with proof of residency. Adequate proof is either a photocopy of your driver's license or a utility bill with your name and street address. Applicants must be at least 18 years of age.
3. Each voucher has 60 day expiration date from the date of issue.
4. You must select your own veterinarian. Veterinarians must bill SOTH directly for the amount on the voucher. You must present the voucher to the veterinarian at the time of surgery. Do not make an appointment for sugery until you receive the voucher.
5. Lost, stolen or vouchers not used within 60 days of issue will not be replaced.
6. Qualifying applicants will be mailed a $25.00 voucher the first week of the month.
7. Voucher applications will only be accepted by mail.
8. Incomplete or duplicate applications without proof of residency will be rejected and will not be returned to the applicant.
